Find a Covid-19 Vaccine Near You

Updated: January 13, 2022

The Supreme Court ruling reinstated, the Centers for Medicare & Medicaid Services (CMS) mandating COVID-19 vaccination for all staff in CMS-certified settings.

The CMS mandate for Kentucky states that all eligible staff at healthcare facilities participating in Medicare and Medicaid programs receive the single dose of Johnson & Johnson or the first dose of either Pfizer or Moderna by Sunday, February 13, 2022. For the vaccines that require the 2-dose series (Moderna and Pfizer), staff must complete the series by March 15, 2022.

The CMS mandate defines and allows staff to submit a medical or religious exemption by February 13, 2022. In addition, this mandate applies to anyone who works at Guardian Angel Staffing Agency, Inc. The CMS mandate requires compliance, which may be vaccination or medical or religious exemption.

Guardian Angel Staffing Agency, Inc., has provided the following options for unvaccinated employees to remain compliant with the CMS mandate:

  1. Become fully vaccinated. Per the CMS guidance, all eligible staff choosing to become vaccinated must receive the single dose of Johnson & Johnson or the first dose of either Pfizer or Moderna, by Sunday, February 13, 2022.
For vaccines that require the 2-dose series (Moderna and Pfizer), staff must complete the series by March 15, 2022. Your assigned facilities have been offering multiple vaccination clinics for all staff to ensure that the COVID-19 vaccine is accessible. You can also choose to be vaccinated at your local pharmacies, Health Department or family medical provider.
Additionally, you can find a Covid Vaccine Location near you at the following website: www.vaccines.gov
  2. You may submit a request for a medical or religious exemption. The emergency regulation requires healthcare facilities to have a process for exemptions based on recognized medical conditions and sincerely held religious beliefs. Exemption request forms will need to be received by Guardian Angel Staffing, for review no later than February 13, 2022. If you would like to request either a medical or religious exemption then please contact Guardian Angel’s Human Resources Department to obtain the appropriate form.

It is not acceptable under the CMS mandate to choose to not become vaccinated against COVID- 19 or receive a medical or religious exemption by February 13, 2022. This will result in separation from Guardian Angel Staffing Agency, Inc.
I hope you understand this is a federal mandate, and that Guardian Angel Staffing, and your assigned facility are required to comply with the CMS mandate.
